I asked director William Sachs if there was additional footage shot for either the nurse scene or Jonathan Demme's death, and he replied:

"The nurse was never planned to be shot.  In my original version, we didn’t know the Melting Man's background, which was added by the producers with the rings of Saturn stuff.  My version started with the nurse running (NOT in slow motion) and crashing through the door.  The first time we saw the Melting Man was with his hand.  The reveal of what he was capable of was the shot of the nurse’s head looking whole until we track around the table to see the eaten side.  This was lost in the producer’s version.  If you watch the documentary and listen to the commentary on the blu-ray, you’ll hear me talk about that.

As to the Jonathan Demme stuff, I don’t remember if it was planned or not.  I also don’t remember shooting it, which was probably due to the lack of time.  That location took a lot of time to light.  Remember, we shot the whole thing somewhere between 14 and 17 days; again, I don’t remember.  We had a lot of nights which required time-consuming lighting and took away from the shooting time."
